O-DSMT is an active metabolite of tramadol and a stronger opioid. It doesn't have any SNRI activity which means you can nod off more + no risk of seizures if you take too much or mix it with other shit
The active metabolite thing is like how codeine works where the high mostly comes from the codeine getting converted to the stronger morphine. Your body can be good or bad at this conversion depending on genetics, which is the reason why codeine and tramadol get some people really high but do jack shit for others.
